Immunization and Insurance Requirements

Upload your Immunizations & Proof of Insurance on myELI

Immunization Requirements

All students at the University of Florida are required to show proof of certain immunizations.

The University of Florida may not accept records in your language from your country, so you must provide your information in English on the official Immunization Form only. The first page explains the requirements and instructions. You can also watch this video to see how to complete the form: https://youtu.be/traB6aoipFM

We recommend that you upload your completed Immunization Form to your application in myELI before you leave your country to begin the review process. Our Student Life Coordinator, Christine Voigt, will contact you if there are any problems with your forms so you have time to complete the requirements before you arrive.

Health Insurance Requirements

Insurance Requirements for F-1 Students

Florida regulations require all students who are attending classes on the F-1 visa to have adequate health insurance for the entire time they are enrolled. All student insurance must meet these minimum requirements. Each semester, you must upload your ID card as proof of insurance on myELI.

Be aware: many international travel insurance plans do not meet these requirements.

If you already have insurance, you must confirm with your company that it meets the requirements and will expire at the end of the term (see Calendar for term dates) or later. If you have questions or need to confirm that a plan meets the requirements, please email our Student Life Coordinator, Christine Voigt.

You should wait to buy insurance until you are inside the US and have registered for classes. For most student insurance plans, you will need to know your UF student ID number to make the purchase.

Insurance Plan Options

The insurance plans below have been reviewed and meet the UF requirements. If you would like more information about any plan, you can contact the company or ask Christine Voigt for help.
